What changed on June 8, 2026

    Business identity

OneNumb is operated by AgentDDS.com LLC. onenumb.com is an assumed business name of AgentDDS.com LLC.

This Patient Terms and Conditions Agreement ("Agreement") is entered into by and between AgentDDS.com, LLC, an Oregon limited liability company doing business as OneNumb.com ("OneNumb," "we," "us," or "our"), and you, the individual booking an appointment through our platform ("Patient," "you," or "your"). By accessing or using the OneNumb.com website, mobile applications, or associated services (collectively, the "Platform") to book a dental appointment, you agree to be bound by this Agreement.

    1. Definitions

    • "Platform" means OneNumb.com, a booking facilitation service connecting patients to dentists.
    • "Dentist" or "Office" means the licensed dental professional or entity providing dental services through the Platform.
    • "Services" means the Platform's facilitation of dental appointment bookings.
    • "No-Show Deposit" means the deposit the Office may charge, via the card you saved on file, if you no-show or cancel less than four (4) hours before your appointment.
    • "Stripe" means Stripe, Inc., the third-party payment processor that tokenizes and stores your card and processes any charge initiated by the Office.
    • "Stripe Connected Account" means the Office's merchant account on Stripe through which the No-Show Deposit is charged directly by the Office (not by OneNumb).
    • "PHI" means Protected Health Information as defined under HIPAA.

    2. Nature of the Services

    ⚠️ CRITICAL WARNING

The Services are intended for non-life-threatening dental issues only. For severe swelling, difficulty breathing, uncontrolled bleeding, fever, or extreme pain — call 911 or go to an ER immediately.

OneNumb.com is a booking service that connects Patients with licensed Dentists. We do not provide dental care, medical advice, or treatment. The Office is solely responsible for any care provided to you.

    3. Booking & Card-on-File Authorization

You are not charged at the time of booking. To reserve your appointment, you authorize the Office to securely store your payment card on file via Stripe using a Stripe SetupIntent (a tokenization-only operation that does not create a charge). By completing booking, you:

    • Confirm the card belongs to you and you are authorized to use it.
    • Authorize the Office to charge the card only (i) as the No-Show Deposit described in Section 4, or (ii) for treatment you separately agree to and authorize in person at the Office.
    • Acknowledge that OneNumb never stores your raw card number; Stripe holds the payment credential as a token.
    • Card re-authorization: Authorize the Office to update or re-tokenize your card via Stripe's automatic card updater service if your card expires or is reissued before your appointment, so the no-show deposit remains chargeable if applicable.
    • Acknowledge that saving a card on file is not itself a charge.

    4. No-Show Deposit

$50 deposit — charged by the Office only on no-show or late cancel

If you fail to attend your appointment, or cancel less than four (4) hours before the scheduled start time, the Office may charge a $50 no-show deposit to your card on file. The deposit is charged directly by the Office through its Stripe Connected Account; OneNumb is not the merchant of record and does not receive any portion of this deposit.

    • On-time attendance: the deposit is never charged.
    • Cancel at least 4 hours before via the link in your confirmation email/SMS to avoid the deposit.

    Dispute path

If you believe the deposit was charged in error, follow these steps in order:

    1. Contact the Office directly within seven (7) days of the charge to request a refund or correction. The Office is the merchant of record and has sole authority to refund the charge.
    2. Request informal mediation by OneNumb if the Office does not respond within seven (7) days of your request, by emailing support@onenumb.com. OneNumb's mediation is informal and non-binding; we do not hold funds and cannot compel a refund.
    3. Card-network chargeback is your final remedy and is governed by your card issuer's rules.

The 4-hour cancellation window and the 7-day dispute window are independent: missing the cancellation window does not shorten your dispute window, and starting a dispute does not waive the deposit if the no-show occurred.

    5. Office Billing & Insurance

OneNumb.com does not bill for or collect payment on behalf of the Office for exams, x-rays, or treatment. All clinical services are billed and collected by the Office directly, including any insurance coordination.

    • Insurance, cash, and card are all accepted at the Office (subject to the Office's policies and your coverage).
    • Prices for exam, x-rays, and treatment are set by the Office, not OneNumb.com.
    • OneNumb.com receives a separate booking-platform fee from the Office. You are not charged that fee.

    6. Privacy and Data Protection

We are committed to protecting your privacy and ensuring the security of your personal data.

    Collection of Information

We collect personal information necessary to provide our Services, including:

    • Name, email address, and phone number.

    7. Liability and Disclaimers

    SERVICE DISCLAIMER

The Platform is a booking tool, not a medical provider. We do not provide medical advice, diagnoses, or treatment. You assume all risks associated with using the Platform.

TO THE MAXIMUM EXTENT PERMITTED BY LAW, OneNumb's total aggregate liability to you, across all claims and incidents combined, shall not exceed $100. This cap is per patient, in aggregate, not per incident or per appointment.

OneNumb is not liable for (i) clinical care provided by the Office, (ii) the Office's billing, refund, or no-show-deposit decisions, or (iii) the Office's Stripe Connected Account, which is governed by the Stripe Connected Account Agreement between the Office and Stripe.

    8. Patient Conduct and Obligations

    You agree not to:

    • Submit false, misleading, or fraudulent information.
    • Use a payment card you are not authorized to use.
    • Interfere with Platform functionality.

    9. Governing Law and Dispute Resolution

This Agreement is governed by Oregon law. Disputes shall be resolved through binding arbitration in Portland, Oregon, under AAA rules.

    10. Acceptance of Terms

By checking "I agree to the Terms and Conditions" and booking an appointment, you certify that you have read, understood, and agree to be bound by this Agreement, including the card-on-file authorization and the no-show deposit policy.

    11. SMS/MMS Messaging Terms

By checking an SMS/MMS consent box on the OneNumb website, users agree to receive SMS/MMS messages from OneNumb related to appointment availability alerts, appointment reminders, account verification, account-related notifications, customer support messages, service updates, promotional offers, product or service announcements, updates, and marketing messages based on their selected opt-in preferences.

Continued participation in the SMS/MMS program after updates means acceptance of the updated SMS terms.

No mobile information will be shared with third party/affiliates for marketing or promotional purposes.
